I just played a coin game where Pebble Beach Hole 4 CTTH#2 was the tiebreaker. Camera views 1 and 2 were identical, both behind the green instead of 1 being an aerial view from between the ball and pin like usual. Also, when clicking on the flag, there was no green slope graphic.

Certain positions on certain holes are just that way.  They either didn't, or couldn't shoot the holes from every possible angle, so you get what you get.  It happens quite often if the green is set tight against a pond or ravine.

PGA #5 is a good example - Par 3 with water all along the left side.  If you happen to miss in or near the fringe on the left side of that green, there was no way for them to shoot from directly behind the player from that angle like you'd normally get in camera view 1 (unless they rented a boat to float around the water I suppose)

I've experienced that too, but I think this is really a bug. This CTTH is a perfectly ordinary approach shot, not a tight shot around the greens. The normal Camera 1 birds-eye view from in front of the green should be feasible, and I would be surprised if it didn't exist in a normal stroke play game.

And the situations you describe just have fewer or unhelpful camera views, right? This shot has two that are identical.
