To all those questions, the answer is yes. I saw a Tour Champion with only about half dozen or so ranked rounds. They're reaching those tiers through winning coin matches.

Similar to above, they're probably playing coin matches. For purposes of calculating averages and stats, coin matches are treated like practice rounds. Nevertheless, I believe win percent in coin matches is being used for determining tier placement.
LOL - Mobile is so different from PC it confuses us PC players! The converse probably applies!
I got nicely called out a while back (no names) for not appearing to play much but, in reality, I was practicing and mapping a lot - which will show I played but shows no games or activity.
